In the past, there was an almost linear relationship between the skills and knowledge of clinicians and patient outcomes. Nowadays, healthcare is a highly complex system. Humans interact with tools, technology, and internal and external organisational factors to produce processes resulting in various outcomes. We must understand and target these dynamic interactions to improve efficiency, well-being and safety.
